• Trying to figure out how to access the renewal? Instructions on how to log in and access the renewal exam will be emailed out separately to all providers who are up for renewal and are available on the APHON website during the open renewal period. • Please note, that although APHON does attempt to contact providers individually to remind them of their expiration date, it is the sole responsibility of the provider to be aware of this date and complete the online renewal. • Renewal Fees : Updated renewal fees can be found on the APHON website. Providers are strongly encouraged to renew early and take advantage of the November reduced fee. • The renewal exam consists of 25 questions and you must achieve a minimum score of 80% to pass. If you are unsuccessful in your first attempt, you will be permitted to retake the exam at no additional cost. If you do not achieve a passing score after two attempts you will be required to take the 2-day provider course again. • If the provider is unable to successfully complete the renewal exam past the expiration date of the card, they must retake the 2-day APHON Pediatric Chemotherapy and Biotherapy Provider Course to have Provider status reinstated. Instruct a minimum of one (1) Provider course per year. • Maintain an average of 2.5 or greater on course evaluations. • Comply with APHON Pediatric Chemotherapy and Biotherapy Provider Course Policies and Procedures. • Maintain APHON membership. • Maintain a current APHON Pediatric Chemotherapy and Biotherapy Provider and Instructor card. • Maintain CPON®/CPHON® certification. • Annually provide the online Instructor Bio/COI form, http://myapps.aphon.org/ConflictOfInterest). • It is the responsibility of the instructor to contact APHON Member Service (847/375.4724) when they have a change of mailing address, email address, last name, phone number, etc. If the above are not maintained, you will not be allowed to teach provider courses until you re- take the instructor course. Your instructor card will become invalid regardless of the expiration date if you do not maintain these items. 13. 90-Day Hardship Instructor Renewal Extension • An instructor who is unable to meet the requirements for maintenance of instructor status may apply to APHON for a 90-day hardship extension. • To be eligible for a 90-day hardship extension, the instructor must contact APHON prior to the occurrence of: o Birth or adoption of a child o Leaving paid employment to care for a child or other dependent o Active military duty o Extended professional absence due to serious illness of self or a family member o Extended professional absence due to the death of a family member o All hardship requests must be made in writing by the instructor. 12.To Maintain Instructor Status •
