8.On-line Evaluation and Continuing Education Credit • Continuing nursing education contact hours (CNE) are provided by APHON to participants who attend the entire course and complete the evaluation form. • The participant does not have to pass the exam to receive nursing continuing education contact hours. • Provider Course: 14 CNE contact hours • Renewal: 2 CNE contact hours If a participant cancels or does not show up for the course, the instructor can return unused manuals to APHON. The instructor will be entitled to a refund, less a 20% restock. A refund request form must accompany the course manuals being returned. • Returned manuals must be in resalable condition. APHON has the right to reject refund requests if the materials are not sent back in resalable condition. • All refunds must be received by APHON within 30 days of original course date. • Refunds will be returned to the original payee. • Instructors also have the option to keep the manuals for future courses. 10.If a Participant Arrives Late or Leaves Early • Instructors must notify APHON immediately, as they should not be given access to the online exam. • APHON will not provide a refund to the course instructors or participant for a participant leaving early or arriving late. • Nursing continuing education contact hours will not be awarded. No partial credit will be awarded. • The instructor decides if the participant can attend a future course. • If the participant attends a future course, the material costs of $95 will be due to APHON. On October 1, of any given year, the provider renewal exam will be open. Email notifications will be sent out regularly to ensure all our providers are able to renew in time. This is why it is imperative that all your contact information is correct and up-to- date. • Updating your information with APHON is simple. There are a few options available to you: o Contact our Member Services Department at 847.375.4886 o By email at info@aphon.org o Update your information on your APHON account online • 9.Participant Cancellations • 11.APHON Pediatric Chemotherapy and Biotherapy Provider Renewal Program • APHON encourages all providers to update their APHON records when the following information changes: address, e-mail, and last name. This can be done using one of the ways mentioned above. • Not sure if you are due to renew? Please refer to your provider card for your correct expiration date. You will be required to log in using your APHON website ID and Password.
