• Payment: Payment for course books is due in full at the time course materials are ordered. Third party checks covering 100% of the cost of the course will be accepted (e.g., from a local APHON chapter or host institution). Acceptable payment methods: Checks issued from a third party, credit card (MasterCard, Visa, American Express, and Discover). Checks from individual course participants will not be accepted. The instructor is financially responsible to APHON for all materials ordered for courses that they teach. Upon order and full payment receipt at least three (3) weeks prior to the course start, APHON will ship The Pediatric Chemotherapy and Biotherapy Curriculum course manualto the instructor via ground transportation, which will arrive at least two days prior to the course. • If the instructor does not receive the course manuals as least two days prior to the course, they must notify APHON immediately for tracking information of the shipment. • Failure to notify APHON in the event manuals have not arrived in time may result in materials not being available for the course. • Please ensure your course manuals were received in good condition immediately . If not, please call APHON immediately at 847/375-4724. In order for APHON to replace your shipment or issue reimbursement, you will need to contact us within seven (7) business days of your shipment being delivered in the cases of damage to materials during the shipping process or missing materials. After seven (7) business days, APHON is no longer responsible. For your convenience, APHON will include a checklist with each shipment so you can personally confirm your order arrived as intended. You do not need to send this checklist back to APHON if there is no damage to your order. 8. Shipping of Course Manuals to Instructors • All course materials provided by APHON are copyrighted and the exclusive property of APHON (course manuals, examination, evaluation, etc.) and cannot be copied or reproduced for any reason. • The course manuals can be distributed to the participant at any time before the start of the course. • The entire course content must be presented, and instruction time must last at least 14 CNE contact hours, not including breaks . The course outline available on the Instructor Only website includes approximate times for content delivery. The instructor is responsible for scheduling breaks and lunch. The course can be taught over 3 to 4 days if needed, as long as all material is covered and 14 nursing hours of instruction are provided. • Additional slides may be inserted as long as the instructor is able to cover objectives and stay within the content delivery time. However, no slides can be removed from the existing PowerPoint presentation. 9. Procedures for Conducting a 2-Day Course •
