APPENDIX 1: APHON Pediatric Chemotherapy and Biotherapy Provider Program Policies and Procedures (updated June 2020) APHON’s Pediatric Chemotherapy and Biotherapy Provider Program was developed to help standardize the education of nurses regarding administration of pediatric chemotherapy and biotherapy. This course does NOT measure competency or provide certification. Th e individual nurse’s institution should determine clinical competency. Instructor names and contact information are available on the APHON web site at www.aphon.org. All instructors must conform to the following program policies and procedures: Instructors will coordinate all aspects of their courses. • ALL instructors must submit their yearly Instructor COI/BIO form. Failure to do so will result in a hold on the course being taught until one is submitted. • At least three (3) weeks before the start your course offering, complete and electronically submit your Chemo/Bio Provider Course Set up Set-Up Online Request Form. 1. Course Logistics • All required information on the course set-up form must be filled out prior to submission. • A Notification is sent to the lead instructor and the APHON Education Coordinator e-mail, notifying both of the successful submission. • Changes to the course dates, instructors, student cancellations, will need to be e- mailed into the Education Coordinator at educationadministrator@aphon.org • Changes to a course can be accepted and processed up to 24 hours prior to the exam. Please note that staff will need a minimum of 24hrs to complete any and all requests. • If this is a web conference course, please indicate so on the online request form. 3. Rushed Set-up Request (Requests after the allotted 3-week deadline) • Course requests after the 3-week deadline will be received but will need to pay an Administrative Rush Fee of $40. o Please be aware that 24hrs min will be needed to finalize any course set- ups. • After confirming receipt of your course set-up request and communicating the rush fee, a Member Services Representative will contact you to collect payment. • Upon confirmation of payment, your course set-up will be finalized and a confirmation of your course registration link will be emailed to the lead instructor for distribution. 2.
