APPENDIX 1: APHON Pediatric Chemotherapy and Biotherapy Provider Program Policies and Procedures (updated June 2020) APHON’s Pediatric Chemotherapy and Biotherapy Provider Program was developed to help standardize the education of nurses regarding administration of pediatric chemotherapy and biotherapy. This course does NOT measure competency or provide certification. Th e individual nurse’s institution should determine clinical competency. Instructor names and contact information are available on the APHON web site at www.aphon.org. All instructors must conform to the following program policies and procedures: Instructors will coordinate all aspects of their courses. • ALL instructors must submit their yearly Instructor COI/BIO form. Failure to do so will result in a hold on the course being taught until one is submitted. • At least three (3) weeks before the start your course offering, complete and electronically submit your Chemo/Bio Provider Course Set up Set-Up Online Request Form. 1. Course Logistics • All required information on the course set-up form must be filled out prior to submission. • A Notification is sent to the lead instructor and the APHON Education Coordinator e-mail, notifying both of the successful submission. • Changes to the course dates, instructors, student cancellations, will need to be e- mailed into the Education Coordinator at educationadministrator@aphon.org • Changes to a course can be accepted and processed up to 24 hours prior to the exam. Please note that staff will need a minimum of 24hrs to complete any and all requests. • If this is a web conference course, please indicate so on the online request form. 3. Rushed Set-up Request (Requests after the allotted 3-week deadline) • Course requests after the 3-week deadline will be received but will need to pay an Administrative Rush Fee of $40. o Please be aware that 24hrs min will be needed to finalize any course set- ups. • After confirming receipt of your course set-up request and communicating the rush fee, a Member Services Representative will contact you to collect payment. • Upon confirmation of payment, your course set-up will be finalized and a confirmation of your course registration link will be emailed to the lead instructor for distribution. 2. Course Set-Up Request Review •

4. Audit Participants: •
There are two types of audit participants, those who are active providers who wish to claim CE, and those who are non-providers. • Audit participants who wish to claim CE will need to pay a $15 processing fee. Payment can be submitted by calling our Member Services Department at 847.375.4724. • Those who wish to audit a class, and will not claim CE will not be assessed a fee as the additional set-up is not required. • INSERT PROCESS FOR AUDIT REGISTRATION • Audit participants are not required to purchase a new curriculum (with the exception of new releases) to attend a course. • Audit only participants do not have access to the post-course exam. • Audit only participants will receive CNE’s after completing the post -course evaluation. • Once evaluations are completed, they will receive a certificate of completion. o Please note, that this certificate of completion in no way extends their certification or counts as a renewal. 5. Online Course Posting andBrochure • When planning your future courses, if you would like to open your course to external registrants, APHON can post the course information on the APHON website. Requests can be made on the Chemo/Bio Provider Course Posting Request Form in the Instructor Community. • A sample course brochure is available on the online posting request form and in the Instructor Community Library. Please be sure to download the most up-to-date brochure each time you plan on distributing as the documents on the Instructor Community website are subject to change. • If you wish to develop your own promotional brochure, one promotional brochure must be developed and submitted for approval to APHON’s Education Manager prior to advertising the first course. The approved brochure may then be used for all subsequent courses. • Instructors may use an online registration system for their course, but they must contact APHON’s Education Manager for approval to use the APHON logo and have the online registration form approved prior to use. Course registration materials include the APHON Pediatric Chemotherapy & Biotherapy Provider Course set-up form, registration link and The Pediatric Chemotherapy-Biotherapy Curriculum course manual. • The completed APHON Pediatric Chemotherapy & Biotherapy Provider Course Set- up form must be submitted three (3) weeks prior to the start of the course. • Once your course set-up is completed, a registration link will be given to you to distribute to your attendees for registration. • Course materials may be ordered through the APHON online bookstore, or by phone. The phone order for course materials and full payment must be received together at least THREE weeks prior to the course by calling APHON Member Services at 847/375-4724. 6. Course Registration •

Additional rush shipping charges will apply for those instructors who miss the three- week minimum deadline for ordering materials. The rush shipping charges must be paid for with a credit card. All rush orders must be placed via telephone to the APHON office at 847/375-4724 with credit card payment. • International Instructors: Please be aware that you are responsible for any and all Foreign Import Tax on any products purchased outside of your country. • If late registrations are added, please order additional materials. An updated registration form must be sent to APHON noting the additions. Additional materials can be ordered (additional rush shipping fees apply) up to SEVEN days prior to the course. • Instructors are required to purchase one copy of The Pediatric Chemotherapy- Biotherapy Curriculum manual for each course attendee. Books may not be shared among attendees or between courses. Every student must have their own individual materials to participate in the course. Curriculums contain an individual validation code in the inside cover which will be used to access the exam by only one user. It is not permissible to allow late registrants to use a book that has already been purchased and used for a previous course. If walk-ins are accepted, the instructor must order adequate course materials prior to the course to allow for walk-ins. If it is necessary for your participants individually to purchase course materials, please contact the APHON Education Administrator. • Whether the instructor is conducting the course for one’s own or ano ther institution, it is the instructor’s responsibility to ensure all APHON policies and procedures are maintained, including timely submission of course registration materials and adherence to all deadlines for submission. The instructor will be the responsible party and point of contact with APHON for all course logistics. • It is preferred that the instructor hosting the course order the course materials. However, if an instructor is having an administrator or someone who is not an instructor take care of this piece, please be sure they are aware of our process to avoid delay in order processing. They will not be given access to the Instructor Only website. Cost: The course participant fee to APHON is $95 per participant. This fee includes The Pediatric Chemotherapy and Biotherapy Curriculum manual, online exam registration, access to online evaluation tool, continuing education credit, provider wallet card (upon passing provider examination), and recordkeeping. The CNE certificate and provider card should be printed and/or emailed to one’s email address following successful completion of the course. The instructor can also make it a requirement that each provider also email them a copy of their card so they can print them for the new provider’s record at their facility. • Instructors may charge additional fees for teaching the course, and/or to cover institution expenses (i.e., meeting room rental, computer lab rental, meals, audio/visual rental).
7. Course Cost and Payment •

• Payment: Payment for course books is due in full at the time course materials are ordered. Third party checks covering 100% of the cost of the course will be accepted (e.g., from a local APHON chapter or host institution). Acceptable payment methods: Checks issued from a third party, credit card (MasterCard, Visa, American Express, and Discover). Checks from individual course participants will not be accepted. The instructor is financially responsible to APHON for all materials ordered for courses that they teach. Upon order and full payment receipt at least three (3) weeks prior to the course start, APHON will ship The Pediatric Chemotherapy and Biotherapy Curriculum course manualto the instructor via ground transportation, which will arrive at least two days prior to the course. • If the instructor does not receive the course manuals as least two days prior to the course, they must notify APHON immediately for tracking information of the shipment. • Failure to notify APHON in the event manuals have not arrived in time may result in materials not being available for the course. • Please ensure your course manuals were received in good condition immediately . If not, please call APHON immediately at 847/375-4724. In order for APHON to replace your shipment or issue reimbursement, you will need to contact us within seven (7) business days of your shipment being delivered in the cases of damage to materials during the shipping process or missing materials. After seven (7) business days, APHON is no longer responsible. For your convenience, APHON will include a checklist with each shipment so you can personally confirm your order arrived as intended. You do not need to send this checklist back to APHON if there is no damage to your order. 8. Shipping of Course Manuals to Instructors • All course materials provided by APHON are copyrighted and the exclusive property of APHON (course manuals, examination, evaluation, etc.) and cannot be copied or reproduced for any reason. • The course manuals can be distributed to the participant at any time before the start of the course. • The entire course content must be presented, and instruction time must last at least 14 CNE contact hours, not including breaks . The course outline available on the Instructor Only website includes approximate times for content delivery. The instructor is responsible for scheduling breaks and lunch. The course can be taught over 3 to 4 days if needed, as long as all material is covered and 14 nursing hours of instruction are provided. • Additional slides may be inserted as long as the instructor is able to cover objectives and stay within the content delivery time. However, no slides can be removed from the existing PowerPoint presentation. 9. Procedures for Conducting a 2-Day Course •

• Case Studies have been designed for optional use during the course. Possible uses of case studies include: 1. Small group interaction with a case study presentation to the entire group. 2. Small group interaction with each group reviewing all cases for discussion within the individual group. 3. Instructor presents each case to participants for group discussion. 4. Instructor presents each case to participants for them to answer on their own in a written format. 5. Participants receive all case studies for essay completion on their own. Instructors whose institutions have live conferencing available are allowed to host a course using this technology. Participants must be able to interact with the instructor and they must participate in the “live course.” Instructors should assign a test proctor at the satellite location who will be responsible for overseeing the exam and evaluation process. The proctor should submit a proctor agreement. The proctor agreement is located on the Instructor Only website. • Instructors who wish to collaborate with another institution using live video conferencing must notify APHON HQ prior to the administration of the course. One Lead Instructor must be identified. The Lead Instructor will take responsibility for submitting the registration roster and ordering all course manuals. • As you may or may not know, our new roster template has an area where a course can be flagged as a “Video Conference” course. The process to set u p the course is the same as previously mentioned in the Roster Submission Overview portion. 6. Video/Web Conference Provider Courses: • Key codes were recently implemented to avoid book sharing. Key codes, as previously mentioned, are pre-printed on the inside front cover of each curriculum. This key code will ensure that each participant has access to their exam at the end of the two-day course. • A Key Code FAQ sheet has been created for instructors. You can find this document saved in the library of the Instructor Community. • The key codes will consist of the following: letters (both upper case and lower case) and numbers. For ease of use and troubleshooting, below are the implemented rules: o No zeroes were used. If it looks like a zero, it is really an uppercase O. o There are no lowercase L's. If it looks like a lowercase L, it is a 1 (one) or uppercase I. o There is no number 5's. If it looks like a 5, it is a uppercase S. • Q: Once someone uses a code, can they re-use it? Once a code is validated and accepted, no one else will be able to use that code. • Q: What if my book does not have a code in it? Please contact APHON to be issued a new key code. • Q: Do I need a new code if I fail my first attempt and need to retake the exam? No, one key code grants you access to both the first attempt, and if failed the second. If you fail the second attempt, you will need to contact APHON for further steps. • Q: I tried using my key code and it says it has already been used, what do I do? Contact APHON when this occurs, we will issue you a new key code. 7. Exam Key Codes: •

• Q: When will I be prompted to input the key code? As soon as you are on the exam portal and have accepted the terms and agreement, the following screen will be displayed:
You will input your code in the box displayed and select Check Code.
7.Online Examination • Participants have three (3) business days to complete the exam if your instructor is not proctoring the exam immediately following the second day of the course. • APHON strongly recommends that the exam is proctored by the instructor(s) but it is not a requirement. • Participants will be directed to log in with a username and password. o If they have created a login with APHON in the past, and registered under this existing account, sign on using those credentials. o If this is their first time logging in, please have them use the email address and password used to set up their accounts and register for the course. Please make sure to have your students test their username and password prior to the exam date to alleviate issues on exam day. • Once they have logged in, they will see a line that says “APHON Pediatric Chemotherapy & Biotherapy Provider Course.” Participants are to click on the graduation cap to the right to access the exam. • Once they have accepted the “I accept” button at the bot tom of the page and have pressed “Continue” (Make sure to read the instructions for the test) the key code screen will appear for them to input the key code printed on the inside front cover of the curriculum. • Once this has been done, they will be prompted to start the test. • Participants have 90 minutes to complete the exam. If they are not finished in the 90 minutes they will be scored based on the questions that were answered in that time. • Once they have completed the exam, they will be prompted to se lect “Score Test” at the bottom of the page. • If a participant has successfully passed the exam, they will are prompted to evaluate the course and the instructor(s). • At this time participants will be able to print out their CNE certificate and Provider Card. • If a participant should fail the first attempt they will be directed to complete the course evaluation and then be asked to begin their second attempt with a new set of test questions. o The second attempt may be started immediately but does not have to be. o The second attempt for the exam will expire in 30 calendar days once the first attempt has been failed. o The participant will have 90 minutes to complete the second attempt. • If a provider fails to log in within three (3) business days for the first attempt the provider will be required to retake the 2-day provider course. • If a provider fails to login within 30 calendar days after failing the first attempt, the provider will be required to retake the 2-day provider course.

8.On-line Evaluation and Continuing Education Credit • Continuing nursing education contact hours (CNE) are provided by APHON to participants who attend the entire course and complete the evaluation form. • The participant does not have to pass the exam to receive nursing continuing education contact hours. • Provider Course: 14 CNE contact hours • Renewal: 2 CNE contact hours If a participant cancels or does not show up for the course, the instructor can return unused manuals to APHON. The instructor will be entitled to a refund, less a 20% restock. A refund request form must accompany the course manuals being returned. • Returned manuals must be in resalable condition. APHON has the right to reject refund requests if the materials are not sent back in resalable condition. • All refunds must be received by APHON within 30 days of original course date. • Refunds will be returned to the original payee. • Instructors also have the option to keep the manuals for future courses. 10.If a Participant Arrives Late or Leaves Early • Instructors must notify APHON immediately, as they should not be given access to the online exam. • APHON will not provide a refund to the course instructors or participant for a participant leaving early or arriving late. • Nursing continuing education contact hours will not be awarded. No partial credit will be awarded. • The instructor decides if the participant can attend a future course. • If the participant attends a future course, the material costs of $95 will be due to APHON. On October 1, of any given year, the provider renewal exam will be open. Email notifications will be sent out regularly to ensure all our providers are able to renew in time. This is why it is imperative that all your contact information is correct and up-to- date. • Updating your information with APHON is simple. There are a few options available to you: o Contact our Member Services Department at 847.375.4886 o By email at info@aphon.org o Update your information on your APHON account online • 9.Participant Cancellations • 11.APHON Pediatric Chemotherapy and Biotherapy Provider Renewal Program • APHON encourages all providers to update their APHON records when the following information changes: address, e-mail, and last name. This can be done using one of the ways mentioned above. • Not sure if you are due to renew? Please refer to your provider card for your correct expiration date. You will be required to log in using your APHON website ID and Password. If you need help signing in, contact our Member Services Department and a representative will be more than happy to assist you.

• Trying to figure out how to access the renewal? Instructions on how to log in and access the renewal exam will be emailed out separately to all providers who are up for renewal and are available on the APHON website during the open renewal period. • Please note, that although APHON does attempt to contact providers individually to remind them of their expiration date, it is the sole responsibility of the provider to be aware of this date and complete the online renewal. • Renewal Fees : Updated renewal fees can be found on the APHON website. Providers are strongly encouraged to renew early and take advantage of the November reduced fee. • The renewal exam consists of 25 questions and you must achieve a minimum score of 80% to pass. If you are unsuccessful in your first attempt, you will be permitted to retake the exam at no additional cost. If you do not achieve a passing score after two attempts you will be required to take the 2-day provider course again. • If the provider is unable to successfully complete the renewal exam past the expiration date of the card, they must retake the 2-day APHON Pediatric Chemotherapy and Biotherapy Provider Course to have Provider status reinstated. Instruct a minimum of one (1) Provider course per year. • Maintain an average of 2.5 or greater on course evaluations. • Comply with APHON Pediatric Chemotherapy and Biotherapy Provider Course Policies and Procedures. • Maintain APHON membership. • Maintain a current APHON Pediatric Chemotherapy and Biotherapy Provider and Instructor card. • Maintain CPON®/CPHON® certification. • Annually provide the online Instructor Bio/COI form, http://myapps.aphon.org/ConflictOfInterest). • It is the responsibility of the instructor to contact APHON Member Service (847/375.4724) when they have a change of mailing address, email address, last name, phone number, etc. If the above are not maintained, you will not be allowed to teach provider courses until you re- take the instructor course. Your instructor card will become invalid regardless of the expiration date if you do not maintain these items. 13. 90-Day Hardship Instructor Renewal Extension • An instructor who is unable to meet the requirements for maintenance of instructor status may apply to APHON for a 90-day hardship extension. • To be eligible for a 90-day hardship extension, the instructor must contact APHON prior to the occurrence of: o Birth or adoption of a child o Leaving paid employment to care for a child or other dependent o Active military duty o Extended professional absence due to serious illness of self or a family member o Extended professional absence due to the death of a family member o All hardship requests must be made in writing by the instructor. 12.To Maintain Instructor Status •
